2.  Double check all information, as it will be printed on the card.

3.  Enclose two color photos (head dimensions 1" X 1") for each ID card.  Do not trim photos.  We will scan  them and make all necessary adjustments.  A clear picture is necessary to make a good novelty ID. All photos will be returned with your filled order.

4.  Send in signed order form and disclaimer along with pictures and payment.  You must read, sign, and return disclaimer along with signed order form for your order to be processed.

5.  Fill in only the information on the order form that applies to the Novelty ID card that you are ordering.   Information that will appear on the different types of Novelty ID is listed next to the Novelty ID example.

6.  All orders without proper payment or information will be discarded.
